Community Health Workers (CHWs)

The Community Health Worker (CHW) serves as a link between Greater Portland Health, health/social services, and the community to facilitate access and improve the quality and cultural competence of health care delivery. This role serves as a cultural broker, assures positive health outcomes, and builds individual and community capacity by increasing health knowledge and self-sufficiency. CHWs role includes a range of activities such as navigation of systems, outreach, community education, informal counseling, social support and advocacy. Staff members are multilingual in French, English, Kinyarwanda, Kirundi, Swahili, and Somali.

Financial Assistance Counseling

Greater Portland Health will never deny access to services for the following reasons: 

  • Because an individual is unable to pay for health care services.
  • Because payment for those services would be made under Medicare, Medicaid, or the Children’s Health Insurance Program (CHIP).
  • Because of an individual’s race, color, sex, age, national origin, disability, religion, gender identity or sexual orientation.

Greater Portland Health offers a discounted/sliding fee schedule based on family size and income.

Interpreter Services

All healthcare appointments and calls can be in your preferred language. Through over-the-phone interpretation, providers and staff can have an interpreter in any language available to support communication. 

 

Peer Support

Are you seeking recovery or in recovery from alcohol or substance use? Are you trying to overcome mental health challenges or other life challenges? If so, you may access peer support services from Greater Portland Health. Our Certified Intentional Peer Support Specialists (CIPSS) are people in recovery who are familiar with a variety of recovery resources in the Greater Portland area. You may access our CIPSS and our peer support services through one-on-one individual support, or in peer support groups. You may meet at one of our locations or out in the community. 

 

Transportation Assistance

Greater Portland Health can help patients get to and from appointments through a variety of transportation options. We offer bus vouchers, taxi rides, UberHealth rides, and collaborate with Maine Association for New Americans who offer rides to healthcare appointments.
